How can I let a custom action be executed at upgrade
please advise which condition I have to set.

I think that you have to query the value of the ActionProperty's in the upgrade table.
Although I'm unsure of the condition that you have to set.
If the Action Property is UPGRADE_1, then I think it would be 'If UPGRADE_1=1' then .......
If you can't figure out the specific's, then I'm sure that someone else can point you in the right direction.
