no icon on start menu on vista

I have a problem the short cut icon on the start menu sometimes changes to the no / can't find icon symbol on some vista machines.

Does anyone know of how to fix this?

Is the shortcut possibly something like a PDF File and the machine doesn't have adobe installed?
Or the shortcut is pointing to an icon/exe/dll file that no longer exists in the location inside the shortcut file?

Could it be our old friend, the icon cache?

It used to be '%SystemRoot%\ShellIconCache' on NT, but changed to '%AppData%\iconcache.db' on XP. I'm guessing that it's in the same place for Vista.

Delete the file, restart the box and the OS will recreate the cache. See if that works.
