Ticket is allowed to be created without selecting a category

My service desk tickets have their category field set to a default value of 'undefined' or blank.  Even though the category field is set to 'always required' and the value is 'undefined', the ticket is still being created.  Has anyone else come across this problem or a fix to this problem?
