What is needed to take our on perm appliance and get it up and running on the cloud?  Do we have to redesign our whole setup or is there a back restore procedure we can follow?

in short:
You simply need to do a backup of the appliance (Settings |Backup) and upload the backup to your AAS-solution (as of today: KAAS as a solution hosted by a partner datacenter for quest or Azure, other solutions are planned) or another environment.
YOu may need to change the network settings afterwards (other IP, webserver name if needed)
