Custom form to submit tickets?

Hello,
We just set up an iMac on the front counter where people come and report their issues. We have a simple form at the moment which submits their name, email and problem they're having. This works and it generates a ticket. Problem is, we're having to edit the ticket and selecting the person who submitted the ticket. Name field is not picking the client's name or the email field. Both name and email address shows up in the comment section of the ticket. 

Has anyone does this maybe using a form? I would like to have the ticket get assigned to the person automatically in Kace.

What are you using to host the form?

If you somehow have the form submit the form via email as the user (email spoofing, of sorts), then it should get attributed to them.

  • We are using google form to submit it. All we have at the moment is name and body but both and body shows up in body of the ticket. - sam240 6 years ago
